Maine RN License or eligible to obtain
5 to 7 years of experience

Our Bangor region has a great position for a registered nurse wanting to make a positive difference in someone's life! Under the direction of the department leader you would be responsible for participaing in multidisciplinary rounds with member organization hospitals, including but not limited to EMMC, Maine Coast and Blue Hill Hospitals to engage in patient discussions and help determine the next level of disposition. You would be considered the subject matter expert on what can and cannot be done in the home-based setting. Additionally, you would be responsible for providing skilled nursing services to pediatric and adult patients in their homes through assessment, and development and implementation of a plan of care, in consultation with physicians and families. Utilizing the latest information technologies and telecommunication systems, we provide you with a variety of nursing experiences, ranging from caring for patients with medical/surgical needs as well as caring for patients with hospice and palliative care needs. In addition you would play an integral role in collaborating with other members of the patient care team.

Diploma or Associate's degree from an accredited school of nursing
Certification in Care Management or other relevant certification
Five years nursing experience, one of which is in home care; or two years home health experience with current nurse preceptor certification
Current ME license to practice as a registered professional nurse
Current BLS for Healthcare Providers
Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions
Valid driver's license with reliable transportation and agency required auto liability insurance.
Ability to travel in assigned service area.
